Baca County Public Health Agency Medical Advisor Agreement
2025 – 2026
I, Dr. Janette Javier agree to serve as Medical Advisor to the Baca County Public Health Agency for year 2025 through 2026. I understand that this appointment and the duties that accompany it involve no compensation of any kind. I agree to serve on a volunteer basis. I agree to generate and establish medical protocols for Baca County Public Health Agency when appropriate and necessary, and I agree to provide medical consultation to the Baca County Public Health Agency when appropriate and necessary. The Baca County Public Health Agency will follow the aforementioned protocol I have established and consult with my office (Southeast Colorado Hospital District / Clinic) when appropriate and necessary. I understand that health, medical, and environmental emergencies may require my attention outside the confines of the normal business day. I will attend and/or be involved in quarterly Board of Health meetings held by Baca County Public Health Agency as Medical Advisor.
