Baca County Personal Care Agency is a Class-B, Non-Medical, licensed agency. We have been providing homemaking and personal care services to Baca and Prowers Counties since 1991. We work with Colorado Medicaid recipients, and also have a grant to offer services for those that do not qualify for Medicaid.

How to Apply

Medicaid Recipients

For clients currently receiving Medicaid, the process to apply is  simple. Visit your local Medicaid office and complete a “Home and Community Base Services” (HCBS)   application. The Medicaid office will determine your eligibility and then refer your case to Options for Long Term Care (OLTC), located in Lamar, CO. An OLTC representative will then be in contact with you to establish your needs, and a referral will be sent to the Baca County Personal Care Agency office. Our office will then be in touch with you to provide you with a Personal Care or Homemaker provider.

Non-Medicaid Recipients

There are Grants and other options for clients that are not receiving Medicaid. For further information on these options, please contact our offices.
